Customizable – One of the unique features to the Coming Home Medical Organizer is the “download” option. As you fill in your personal information it will expand to fit your personal needs. Each form will be specific to you and your family’s health needs.

Flexible – Directly type your health information into each form. You can also just print the PDF version documents and then hand-write your information. Our forms can also fit to the A4 format.

Portable – Make as many copies of each form as you would like. Print extra copies of completed forms to take with you when meeting new doctors, specialists, or other medical team members. Organize your completed Coming Home forms into your own 3 ring binder. Continue to add medical information and update as needed.

Multiple Formats – The CD includes all 18 forms in word format, PDF format, and A4 format

Multiple ways to use it

Personal Health Record/Tool – One way to use it is to establish baseline information. This is very important for answering questions like, “when”, “how long”, “what”, etc.

Doctor/Specialist Visit – Every visit is met with the same questions being asked, “What medications are you taking?” “Do you have any allergies?” Take a minute to write down the information and have it readily available for your appointments.

Emergency Room Visit – No one is ever “ready” for an ER visit, but you can be prepared with the critical information that is needed in a crisis moment. Having this ahead of time will save you precious time and money in the long run.

Health information available when connecting with government agencies – the time it takes to fill out all of the necessary paperwork needed to apply for assistance, seek a secondary insurance, and estate planning for your family will dramatically decrease as you gather your personal health records and information together.

Education information available when needed for school – If your child has an IEP, Individual Education Plan, a 504, or other forms necessary for their education this will help to keep it all in one place, list out their therapy schedules, class schedules, bus drivers, etc.

Personal/Home life snapshot of daily care – As you create a daily schedule for your loved one this is helpful for when you have a nursing agency provide staff to help you at home. They can get a glimpse of how you structure their day, school, work, therapies, even when they watch their favorite TV show.
