Make a list of the medical professionals involved in your loved-one’s care. Write down their name, specialty, contact information, and hospital where you see them at. You will want to include doctors, specialists, nurses, therapists, case workers, teachers, and aids.
Work together as a partner with your loved ones medical team. Each step you take will bring you closer to becoming a collaborative partner involved in your loved ones plan of care.
Taking this step will create a circle of care team for you and your loved one. Surround yourself with a team of family, friends, faith community, and medical professionals to rely on for strength, support, and encouragement.
