A New Change
Change happens when you least expect it. Whether you have suddenly received a medical diagnosis, are facing an unexpected surgery or are having to care for a loved-one; you now must take steps to create your “new normal”.
One of the main obstacles to getting yourself organized is the problem of “information overload”. There are layers upon layers of detailed information surrounding chronic illness and disability. Your first step is beginning the process of understanding the personal health information for you or your loved-one which will keep you from drowning in the sea of information.
Medical changes happen when you least expect them. If you think about it, there is a pattern of questions that your doctor asks every time you see them. The information you provide is usually the same – what is your date of birth? What is your contact information? Do you have any allergies? What is the medication list?
Sometimes, you have so much medical information to remember that the 20 minutes that you have with your doctor is spent trying to remember what medicine you are taking, when you last saw the specialist or your new health insurance provider, etc.
Once you get past these questions then you are free to talk about why you are there.
By having this information already written down you have “freed up your mind” to focus on what you want to get out of the doctor’s appointment.
- Current Status
The current status form is a picture of your loved one. It is a brief snapshot of your loved one and their current health. It is fully detailed yet contained to one page. It highlights the diagnosis, medications, primary doctors, allergies, and contact information. - Medication List
Create a list and keep track of medications, vitamins, herbals, and supplements. You will want to note where you get your medication, the address and phone number of your pharmacy, whether you order online, and if you need a doctor’s script for reorders.
